Important Things to Know for 2012-2013

Plan Year September 1, 2012 to August 31, 2013
  • NEW National Benefit Services will be the new Section 125 Third Party Administrator (TPA) for Shallowater Independent School District. A new NBS Flex Card will be issued to those Districts who participate in the Flex Cardoption. Direct deposit is available. Please remember to keep your receipts if you use your Flex Card. The medical reimbursement annual maximum is $2,400 / plan year and the dependent care reimbursement annual maximum is $5,000 / plan year if married filing jointly and $2,500 / plan year if filing single. As a result of the Health Care Reform Act, the maximum annual contribution for your Medical Reimbursement will be $2,500 starting September 1, 2012.

    If you currently participate in the medical or dependent care reimbursement flexible spending accounts, you MUST log in and elect this benefit every year. If you do not re-enroll annually, your participation will automatically be waived.

  • NEW Cigna Dental Plan Two Passive PPO plans will be offered a High Plan and Low Plan.Both plans will have a $1,000 Calendar Year Maximum,$50 deductible / $150 Family max and both plans offer Lifetime Maximum of $1,000 for Orthodontia for dependent children up to age 19.

  • NEW Cigna Basic Life & Voluntary Term Life/AD&D Shallowater Independent School District provides eligible employees with $30,000 of Basic Life / AD&D coverage as an employer paid benefit. Voluntary Term Life will be offered on Guaranteed Issue basis (no health questions asked) for employees up to $150,000, spouse up to $75,000 and $10,000 for dependent children. Plan is portable should you leave the district, you can take it with you. AD&D coverage is offered for employee only and family coverage.

  • NEW Med-Link Insurance by American Public Life Medical Gap insurance is designed to cover any out-of-pocket deductible amount, and out-of-pocket co-payment or coinsurance amounts the Covered Person actually incurs after the Medical Plan has paid. There are two in-patient benefits availableto choose from. The $1,500 in-patient benefit and $2,500 in-patient benefit availableto employees that are enrolled in the Active Care 1 and Active Care 1HD. This planalso pays $200 outpatient benefit and $25.00 Physician Outpatient Benefit. Thereis a maximum of $125 per family for the Physician Outpatient Benefit.
