Important Things to Know for 2010-2011
- The Shallowater ISD Cafeteria plan year is September 1, 2010 through August 31, 2011.
- MGM is the Third Party Administrator (TPA) for Shallowater ISD for plan year September 1, 2010 through
August 31, 2011. Direct deposit is available
- New Dental Options Effective 9.1.2010. Dental: Guardian is the Dental provider for Shallowater ISD.
Guardian Dental is now offering the Network Access Plan and the Value Plan. The Network Access Plan is a
Passive PPO which means you can go to any dentist you choose. The Network Access Plan reimburses 100% for
Preventative, 80% for Basic Services and 50% for Major Services and offers a $1,000 plan annual max. With
the Value Plan, members must go in network but receive a higher reimbursement percentage. The Value Plan
reimburses 100% for Preventative, 100% for Basic Services and 60% for Major Services and offers a $1,000 plan
annual max. Both plan options also pay up to $1,000 lifetime max for Orthodontia for children under the age
of 19.
- Vision: Block Vision is the Vision provider for Shallowater ISD. To view plan summary and rates,

- Disability: Unum Provident is the disability provider for Shallowater ISD. Employees can enroll on a
guarantee issue basis (no health questions asked). Guarantee issue amounts are subject to pre-existing

- Disability: Unum Provident is the disability provider for Shallowater ISD. Employees can enroll on a
guarantee issue basis (no health questions asked). Guarantee issue amounts are subject to pre-existing

- Accident: Loyal American is the Accident provider for Shallowater ISD. To view plan summary and rates,
click on Benefits and Forms/Accident.
- Effective 9.1.10 all employees of Shallowater ISD can enroll or increase their coverage on a Guarantee
Issue Basis. This means no health questions asked. Voluntary Life: 5 Star is the Voluntary life provider for
Shallowater ISD. Employees can enroll for 7 times salary up to $150,000 for self, 50% of employee election
up to $75,000 for spouse and $10,000 for dependent children on a guarantee issue basis (no health
questions asked). Employees can also purchase AD&D coverage up to the amount of life insurance they
purchase on themselves (AD&D not available for spouse/children).
- Individual Life: 5 Star offers an individual life insurance plan to age 100. Employees can purchase
policies on self, spouse or children and grandchildren and policies are renewable and premiums stay level to
age 100

APL Cancer, Loyal American Accident, 5 Star Voluntary Term Life and AD&D, 5 Star Individual Life and Flexible
