Important Things to Know for 2011-2012
- The Shallowater ISD Cafeteria plan year is September 1, 2011 through August 31, 2012.
- MGM: Will the Third Party Administrator (TPA) for Shallowater ISD for plan year September 1, 2011 through August 31, 2012. Direct deposit is available, click on the Benefits & Forms/Reimbursement to locate form.
- Dental Plan: Guardian will remain the Dental provider for Shallowater ISD. New Dental rates will be effective 09/01/2011. Guardian Dental is offering the Network Access Plan and the Value Plan. The Network Access Plan is a Passive PPO which means you can go to any dentist you choose. To view plan summary and rates, click on the Benefits & Forms/Dental.
- Vision: Block Vision will remain the Vision provider for Shallowater ISD. b> New Vision rates will be effective 09/01/2011. Co-oays: $10.00 eye exam co-pay and $25.00 eyewear co-pay. To view plan summary and rates, click on Benefits and Forms/Vision.
- Disability Plan: Unum Provident is the disability provider for Shallowater ISD. Employees can enroll on a guarantee issue basis (no health questions asked). Guarantee issue amounts are subject to pre-existing condition exclusions. To view plan summary and rates, click on Benefits and Forms/Disability.
- Group Cancer: American Public Life (APL) will remain the cancer provider for Shallowater ISD. Employees can enroll on a guarantee issue basis (subject to a 12 month pre-existing condition limitation). To view plan summary and rates, click on Benefits and Forms/Disability.
- Accident Plan: Loyal American will remain the Accident provider for Shallowater ISD. To view plan summary and rates, click on Benefits and Forms/Accident.
- Voluntary Life: 5 Star offers Voluntary Life for Shallowater ISD. Employees can enroll for 7 times salary up to $150,000 for self, 50% of employee election up to $75,000 for spouse and $10,000 for dependent children, this coverage requires a paper application. Employees can also purchase AD&D coverage up to the amount of life insurance they purchase on themselves (AD&D not available for spouse/children). - Individual Life: 5 Star offers an individual life insurance plan to age 100. Employees can purchase policies on self, spouse or children and grandchildren and policies are renewable and premiums stay level to age 100
Benefit plans for 2011-2012 plan year include: Guardian Dental, Block Vision, Unum Educator Select Disability, APL Cancer, Loyal American Accident, 5 Star Voluntary Term Life and AD&D, 5 Star Individual Life and Flexible Spending Accounts.
